The final diagnosis is made by prostate biopsy, which should be performed if prostate cancer is suspected based on a digital rectal exam, regardless of the PSA level. Similarly, if the PSA level is greater than 4 ng/ml, a biopsy is indicated even if the digital rectal exam is normal.
A biopsy involves taking tissue samples from the prostate, which a pathologist analyzes microscopically. The pathologist's findings reveal whether cancer is present, where in the gland it is located, an estimate of its size, and the malignancy grade of the tumor cells. If the pathologist does not find tumor tissue, but suspicion of a tumor remains based on their findings or subsequent follow-up examinations, the biopsy needs to be repeated, sometimes several times.
